As an RN on the Home Care Team, you will help provide coverage for patients living closer to eastern Cuyahoga County and Lake County. Here you will deliver the best possible care to patients in the comfort of their own homes. In this role, you will provide assessments, complex treatments and education to help avoid readmissions to the acute care setting and ensure comfort.
