Hospice On Call RN — Where Support Meets Impact
+ Lower patient-to-nurse ratios for meaningful, unrushed hospice care
+ Strong interdisciplinary team support— collaborative care with Social Work, Chaplaincy, Aides, and providers, so you’re never practicing alone
+ Leadership that understands hospice nursing
+ Dedicated internal after-hours triage team to support nurses outside of regular business hours
+ Homecare Homebase (HCHB) EMR for streamlined documentation
Office Location: 1028 S. Main Street Suite C Fond du Lac, WI 54935
Coverage area: Travel throughout the territory is required
Schedule: Full Time On Call Friday 4PM- Monday 8AM
