Key Responsibilities
+ Provide direct nursing care in the patient’s home
+ Administer medications and treatments as prescribed
+ Monitor and document patient condition and vital signs
+ Perform skilled nursing procedures (e.g., trach care, ventilator management, G-tube feeding, wound care)
+ Collaborate with physicians, families, and care teams
+ Educate patients and families on care plans and disease management
+ Maintain accurate and timely documentation
