Job Description
Assist Head of Department in ensuring that the department’s performance indicators are met. Assume leadership to projects and portfolios with professional/organizational implications. Provide clinical consultation and support to complex casework requiring intervention at system level. Clinical supervision and mentoring.
Primary Responsibilities And Duties
- Assume leadership to key projects with professional and organizational implications, e.g. LOS Task Force.
- Supervision on Casework: work on case assessment and action plan. Preliminary case assessment is to be completed by day 2–3 of referral. Complete assessment and action plan is to be completed by day 3–5 of referral. Case progress is to be regularly reviewed until patient is discharged home. For patients who we anticipate requiring continued support after discharge from hospital, post‑discharge follow up is required until the social situation is managed and stabilized.
