The Senior Program Delivery Professional manages and executes on all Stars, Clinical, and Pharmacy projects for the In-home Health and Well-being Assessment Team. The Senior Program Delivery Professional work assignments involve moderately complex to complex issues where the analysis of situations or data requires an in-depth evaluation of variable factors.
The Senior Program Delivery Professional executes on projects designed to improve clinical quality tied to the in-home assessment program:
+ Continues to deliver existing Stars programs included in IHWA program
+ Partners directly with Stars, Clinical, and Pharmacy partners to identify and prioritize opportunities to incorporate additional clinical value
+ Monitor and lead oversight of all in-home visit data collection, ensuring real-time visibility and compliance with policies and Stars requirements
