Responsibilities
- Conduct comprehensive and advanced bio-psycho-social-spiritual assessments for residents, especially those with complex or high-risk care needs.
- Manage a caseload of complex cases through systemic assessment, individualized care planning, intervention, and ongoing review.
- Provide individual and family counselling to support adjustment to institutional living, manage grief and loss, and cope with illness, disability, and end-of-life issues, including facilitation of Advance Care Planning (ACP).
- Lead the assessment and facilitation of applications for national financial schemes and subsidies (e.g., Eldercare, Medifund).
- Coordinate and oversee discharge planning, ensuring appropriate community linkages and support systems are in place for residents’ safe transition to home or other care settings.
