Summary This Women's Health senior social worker is assigned to PACT team & is located at the Community-Based Outpatient Clinic(CBOC) with responsibility to provide direct services for veterans & their families. This position coordinates outreach, interdisciplinary activities, psychosocial assessments, treatment planning, consultation/education to Health Care System staff, collection and maintenance of statistical data, program planning evaluation, modification, & supervision of students as assigned.
