Job Description
Reporting to the Manager, you play a critical role in the care of our patients.
Responsibilities- You will provide psycho-social support for patients and their families, including psycho-social and financial assessment, casework, counselling and home-visits and work closely with other multi-disciplinary professionals. You are required to liaise with community service organisations and agencies.
- Degree in Social Work
- Registered with the Singapore Association of Social Workers
- At least 3 years of relevant experience in a healthcare environment
- Bilingual and ability to communicate in local dialects preferred
- Passion to serve and provide good patient care
- Good interpersonal skills and able to work independently
- Flexible, adaptable and able to work effectively in varied setting
- Good in analytical and interviewing skills
