Job Summary
Reporting to the Day Care Manager, you will be responsible for providing excellent individualized rehabilitative care to patients in Day Care, Home Care and Inpatient to optimize their quality of life.
Job Description- Manage clinical caseload and support palliative rehabilitation practice (e.g. breathlessness management, prescription of equipment, collaborative goal setting) across the various care settings.
- Deliver evidence-based individualized care through holistic comprehensive assessment, treatment and evaluation of care.
- Work collaboratively with multidisciplinary colleagues to meet patient needs.
- Participate in service review to drive service improvement.
- Contribute to implementation and monitoring of adherence to relevant clinical guidelines.
