As a Support at Home Service Partner, you will be the primary point of contact for our participants in our local LGA`s. The role requires exceptional communication, organisational skills with experience working in the aged care sector.
You will be responsible for facilitating participant access to services and providing administration support to the Support at Home team. You will liaise with our Support at Home participants, internal and external stake holders, and suppliers to ensure our participants care needs and goals are met within appropriate time frames.
You will be responsible for:
- Providing high level of customer support to Support at Home participants and team
- Liaising with Support at Home participants and all involved in their care and internal and external providers to arrange and manage support services in the participant’s home and community
