Our client is seeking a Social Care Manager to support members with ongoing Health-Related Social Needs (HRSNs) through care coordination, eligibility assessments, and ongoing support services. Responsibilities
- Manage incoming referrals for social care and care management services
- Conduct outreach, screenings, and eligibility assessments for Medicaid members
