Social Work Coordinator

Join a compassionate healthcare social work team supporting patients and families through financial assistance, discharge planning, and care coordination. This role provides operational and administrative support while working closely with Medical Social Workers to enhance patient care and service experience.

Support the daily administrative and operational functions of the department, including frontline counter duties.

Conduct intake interviews, financial assessments, and assist patients with financial assistance applications.

Support discharge planning and coordinate referrals with internal and external care providers.
