Liquid Personnel is seeking an Social Worker to join an esteemed client’s Adult Reablement Team.
Responsibilities :
In this role, you will
Hold a short-term caseload within a reablement service, monitoring progress, completing Care Act assessments, and responding to safeguarding concerns.Carry out formal mental capacity assessments, prioritise and review cases, and offer information, advice, and signposting where appropriate.Work in a strengths-based, person-centred way with individuals living with varying levels of risk, using creativity and flexibility to support outcomes.Contribute social work expertise within a small, supportive team and wider multi-disciplinary service, supporting colleagues, apprentices, and shadowing.<...
