Health Advocates Network is currently seeking a **Social Worker** to work at a facility in **Poughkeepsiee, NY. ** These are _registry_ positions with our company. **$100 Sign-On Bonus**
**Pay Rate: ** $45 / hour
**Shift details:**
+ Monday- Friday 7:30am -4:00pm
Job Descriptions:
• Maintain a caseload of 6 children in the IDT program within the elementary classroom, kindergarten - 6th grade
• Participate in IDT admissions with child, family, MD, and school liaison
• Provide Weekly Individual Therapy
• Provide Daily Group Therapy
• Provide Family Therapy and Crisis Intervention as needed
• Facilitate Transition Meetings for each student on their caseload that include the student and family, IDT teacher, school liaison and other professionals from the student’s home district
• Collateral involvement with participating schools and agencies
• Function as part of an interdisciplinary treatment team
