What will your responsibilities be?
As a Social Worker in the Cognitive and Physical Disabilities Team, you will be responsible for conducting thorough social care assessments to identify the needs of adults with cognitive and physical disabilities. You will develop personalised care and support plans in collaboration with individuals, their families, and other professionals while applying a strengths-based approach to promote independence and well-being.
