Social Worker Opportunity:
- Full-time, permanent, Monday-Friday schedule
- Provide direct support and case management services to assigned clients
- The Social Worker will monitor and improve the patient experience by focusing on quality, timeliness, and continuity of care
- The Social Worker will manage a varied clinical caseload serving all age demographics from pediatric to geriatric populations
Social Worker Qualifications:
- Must be a Licensed Clinical Social Worker (LICSW candidates are highly favored)
- Background in clinic setting is preferred, but not required
