**Job Summary and Responsibilities**
The **Social Worker, MSW** provides comprehensive psychosocial assessment, intervention, and discharge planning services to patients and their families within an acute care hospital setting. Utilizing a patient-centered, strengths-based approach, the Social Worker addresses personal, social, and emotional factors impacting a patient's health status and recovery. This role collaborates closely with the interdisciplinary healthcare team to ensure continuity of care, optimize patient outcomes, and facilitate safe and appropriate transitions post-hospitalization. The Social Worker advocates for patients, empowers them, and connects them with necessary resources to achieve their highest level of functioning and well-being. Education patients, families, and medical staff regarding available resources and guidelines associated in accessing resources. Provide applications as needed for these resources.
