Social Worker (Progressed & Non-Progressed)
An exciting opportunity has become available to join the Short-Term Services Team. We are currently recruiting for two full-time positions and one part-time position. The part-time role is for 0.8 FTE (29.5 hours per week).
These vacancies are open to both progressed and non-progressed Social Workers.
About you
The Short-Term Services teams are aligned with Leighton and Macclesfield Hospitals and work in close partnership with Mid Cheshire Hospitals NHS Foundation Trust and East Cheshire NHS Trust. The service undertakes proportionate assessments to support timely hospital discharge, working alongside hospital therapists to assess short-term care and support needs, and developing person-centred recovery and support plans with individuals and their families or carers, including Reablement and equipment provision.
