Job Description
Job Responsibilities
- Engage identified students regularly to provide mentoring and emotional support
- Build positive relationships with students and contribute to their holistic development
- Plan and conduct student-centric interventions (outreach, home visits, family sessions)
- Facilitate group-based programmes for students with social, emotional, or behavioural needs
- Identify student concerns and escalate cases to school leaders and support teams
- Participate in case review meetings with counsellors, SEN officers, and school personnel
- Support referrals to external agencies and community services when required
- Maintain accurate case notes, documentation, and progress updates
