What You'll Do
As a Social Worker you will participate as a member of the admissions team and provide client counseling, complete social service assessments and treatment plan maintenance. You will provide a range of education and planning for clients, families and staff.
Your Responsibilities Will Include:
+ Complete social service assessments (to include providing client information on Advanced Directives) within required timeframe of admission as ordered by referring physician.
+ Participate in the development and maintenance of the master problem list and social work treatment plans, and identify appropriate short and long-term social service goals for clients.
