Key Responsibilities
- Support care integration work in Toa Payoh subregion with partner organisations, including SCC, AAC, HPC+, and HT providers, to achieve deliverables specified for the Integrated Community Care Provider.
- Work with ground teams to implement the sub‑regional workplan, and monitor and report on the progress.
- Coordinate and facilitate regular meetings and check‑ins for partner organisations to build understanding and work through implementation and other challenges.
- Engage and build networks and partnerships with other community care providers, regional health systems, grassroots organisations and other stakeholders.
- Assess resources and identify opportunities for care integration and coordination collaborations across ICCP partners and with external partners.
