Responsibilities
- Provide skilled speech-language therapy services to patients in their homes
- Evaluate, diagnose, and treat speech, language, communication, and swallowing disorders
- Develop and implement individualized treatment plans
- Collaborate with an interdisciplinary healthcare team to ensure patient-centered care
- Maintain accurate and timely electronic documentation
Requirements
- Active New York State Speech-Language Pathologist license
- Current BLS/CPR certification
- Home care experience preferred
