Job Responsibilities:
- Conduct initial patient assessments and utilise appropriate outcome measurement tools to evaluate functional status and therapy needs.
- Develop and implement individualised care plans and rehabilitation goals tailored to each patient's condition and progress.
- Provide physiotherapy treatment and intervention in home-based care settings.
- Educate and train caregivers on patient care techniques, exercises, and use of prescribed equipment.
- Support discharge planning and coordinate referrals to other healthcare or community services where necessary.
- Participate in monthly home therapy meetings and multidisciplinary discussions.
- Conduct therapy in-service training sessions and attend departmental training programmes for continuous professional development.
