Role Overview
The Reablement Assistant will work as part of the SH&C Reablement Unit, providing care, support and rehabilitation for patients within the Unit and in their own homes. The post holder will undertake delegated tasks, follow care plans, and report daily to the Nurse in charge and therapy leads. The role is designed to develop the assistant’s skills and abilities to work with this client group and provide the appropriate level of reablement.
Responsibilities
- Provide care, support and rehabilitation for patients within the Unit and at home.
- Undertake delegated tasks and follow care plans.
- Report daily to the Nurse in charge and therapy leads.
- Develop skills and competencies aimed at meeting patient needs.
- Collaborate with clinical teams to determine further training and development.
