The Adult Social Care Teams support adults to maintain their health and wellbeing, and maximise independence, choice and control.
The Hospital Social Care team is located in Dewsbury District Hospital and supports adults with complex health and social care needs. The team provides assessments, support packages and early reviews to Kirklees residents that have been admitted to hospital to facilitate timely discharge.
The Role
As a Team Manager, you will lead and manage a team of staff including qualified Social Workers. Hospital discharge is a fast‑paced environment, and the team works to clearly defined targets and deadlines. Consequently, you will play a key role in ensuring the timely completion of assessments and effective discharge planning. This will also involve you developing positive, supportive relationships with our partner agencies, including the acute hospital trust.
