Job Description
Location: Multiple locations / West
Duration: 1 year renewable
Hours: 5.5 days, office hours
- Build strong relationships with patients, caregivers, and healthcare professionals
- Coordinate and support patient care activities using healthcare IT systems
- Track, document, and monitor patient care interventions and clinical indicators
- Perform basic health parameter measurements (e.g. blood pressure, weight, glucose monitoring)
- Support national preventive health screening and vaccination initiatives
- Provide basic health education on lifestyle management, physical activity, and healthy living
- Promote social prescribing and increase awareness of community resources available to patients
- Educate patients on the use of health monitoring devices and vaccination requirements
