Job Description

We are passionate about helping kids thrive at home and are looking for a Transitional Care Nurse who shares that passion to help transition medically-fragile children from the in-patient to home setting.

THE POSITION

  • Location: Omaha, NE
  • Title: Transitional Care Nurse
  • Schedule: Full Time
  • Shift: Flexible to work avariety of days and shifts around the Omaha metro area, to support the unique needs of patients transitioning to the home setting. When possible, schedules will be created 2 weeks in advance. Differentials are available for night and weekend shifts.
  • position will support a patient and family as they transition to the home setting for the first time, after a hospitalization or new diagnosis/technology.
