We’re seeking an experienced Travel Registered Nurse – Case Manager to coordinate patient care, improve outcomes, and guide patients through every stage of their healthcare journey. This role is ideal for an RN who thrives in a collaborative environment and excels at planning, communication, and advocacy.

• Coordinate and manage patient care plans across the continuum of care


• Evaluate patient needs using evidence-based criteria and care management guidelines


• Facilitate appropriate utilization of services and level-of-care decisions


• Develop and implement safe, effective discharge plans
